Youth can sign themselves us up or a parent/guardian can sign you up. Use this form if you are doing this project on your own (that is, your teacher is not submitting the project for you). Please note that you will use this form to sign you/your child up, but you will need to also submit the parent consent form. (That form is different because it allows us to share your child's work publicly).
